I have come across the following death of Charles Richard Wardle born 1884 Rudyard near Leek, son of Enoch Wardle and Hannah nee Heath. Died the 20thSeptember 1917, at Queen's Hospital Birmingham.

Charles Richard Wardle, aged 33, a soldier patient at the Queen's Hospital suffering from shell shock has died at that institution from injuries to the head sustained falling from a window.

On the death certificate Charles Richard suffered a fractured head. The coroner stated an accidental death.On Charles Richard's military records it states that he committed suicide.
